Common causes

  • The account was created with SystemProgram.createAccount but InitializeAccount was never called.
  • The associated token account for this wallet and mint does not exist yet.
  • A previous instruction in the same transaction that should have initialised it failed or was omitted.

How to fix it

  • Create the ATA first (idempotently) in the same transaction, then transfer.
  • Check getAccountInfo on the address: if data is empty or the owner is the System Program, it is not initialised.
